Type: Per Diem | Part-time

Salary: $75 - $90 per hour

Based in San Ramon, our team is growing, and we are seeking a dedicated Physical Therapist (PT) to join us. In this role, you will deliver skilled in-home therapy services, design personalized treatment plans, and work collaboratively with caregivers and clinical staff to help clients restore mobility, regain independence, and improve their overall quality of life.

Key Responsibilities

• Perform initial evaluations and re-assessments to determine mobility, balance, and functional needs
• Develop and implement customized treatment plans aligned with patient goals
• Deliver gait training, range-of-motion exercises, and fall prevention strategies
• Educate patients and caregivers on home safety, adaptive techniques, and equipment use
• Collaborate with PTAs, RNs, OTs, and interdisciplinary team members during care coordination
• Accurately document services in the EMR (WellSky) per Medicare guidelines
• Monitor patient progress and communicate changes in condition to the care team
• Supervise PTAs and aides as needed

Qualifications

• Active, unrestricted California Physical Therapist license
• Doctor of Physical Therapy (DPT) degree from a CAPTE-accredited program
• Successful completion of NPTE and California Law Exam
• Minimum 3 year of clinical experience ( home health or outpatient preferred )
• Current CPR certification
• Proficient in electronic documentation (WellSky, Kinnser, or similar)
• Reliable transportation and valid California driver’s license
• Excellent interpersonal and clinical reasoning skills
